First open the File menu, select New, and select Web page from the submenu. Click the General tab in the pop-up dialog box and select the Form Page Wizard and click OK.

After the Form Making wizard dialog box, click the Next button, and when we first use the Form Page wizard, we need to click the Add button to add the problem. If you also want the form to be submitted by a visitor to your mailbox by email, just fill in your email address in the text box after the email address, and then click the OK button.
