1. make sure the problem is repeated in different browsers of different machines.
2. install the software on another server and test whether the software is repeated (skip if it is difficult)
3. if 1 (2) is true, create a simple code that can repeat the problem. it should include at least your form/submit post-processing php file.
(Remove irrelevant html/php code or confidential code from your code and test it to confirm that the problem still exists. if the problem disappears after some code is removed, check this part of code)
4. install the simple code version on another server and test it to confirm that the problem still exists.
5. paste the code
6. if 5 cannot be implemented, add echo or error_log to the code and observe the differences between the logs when the submission is successful or fails.
7. If no difference is found in 6, add more logs.
8. Repeat 3 or 7 until the cause is found.
9. if you still cannot find the cause, please reply to the discussion and provide more information
