I. Mode
There are no more than two development modes for the electronic medical record system in China: the C/S mode and the B/S mode. The C/S mode is generally developed based on an open text editor, b/S generally uses the page filling mode and OCX control mode (this mode is actually a C/S mode ). The two models have their own advantages and disadvantages. From my personal point of view, they tend to be C/S (here it refers to the C/S with a three-tier structure) for the following reasons:
1. The biggest advantage of B/S over c/s is that the client is maintenance-free. In fact, C/S is ready, and automatic upgrade and maintenance-free client can be realized, my previous medical insurance system in a region adopts a three-tier structure. Over 1000 C/S clients on the Wide Area Network are not maintenance-free.
2. c/s has better interface operability, especially for implementing more complex interface operations, which can better implement natural language input and structured processing to a certain extent. Due to the limitations of the editor function, B/S lacks operations between texts, data elements, various prompts, and drawing processing.
3. c/s has a more intuitive interface to implement WYSIWYG writing, which is difficult for B/S to do without controls.
4. C/S has better efficiency, especially when processing and displaying a large amount of data, C/S has more obvious advantages. The B/S mode is restricted by servers and browsers, and the aging rate of a large amount of data is decreased linearly. We have tried IE to display 1000 records at the same time, which is quite slow.
Of course, the specific mode used depends on the product route and technical precipitation. it is best to adopt the same mode as his. Because B/S electronic medical records must rely on databases, it is difficult to see normal channels. They can be seen in a rush only in user hospitals, the following describes the electronic medical record editing controls in C/S mode.
Ii. Functions
In C/S mode, the core of the electronic medical record system is the electronic medical record editor control. The functional requirements of the electronic medical record editor are described in many articles.
The functions of the electronic medical record editor are described as follows:
1. Writing: This is the most basic requirement, with few details. According to the habits of clinicians, it is best to see what is what we get, in addition, record-filing is essential to the writing function.
2. Data Element Management: according to the requirements of the Ministry of Health, the electronic medical record system must be able to structure the medical record content and manage it according to the data standards of the Ministry of Health. The purpose of the electronic medical record structuring is:
A) quality control requirements: the system must be able to identify and distinguish the medical records, and perform operations such as writing restrictions, timeliness reminders, and automatic scoring of medical records based on different content.
B) Teaching Requirements: writing medical records not only records the medical process, but also plays an important role in training the clinical thinking model of young doctors. Therefore, the electronic medical record editor should be able to guide writers to complete the writing process in the correct way of thinking.
C) Scientific research requirements: clinical research is to obtain historical medical data from medical records, and medical record retrieval is an important means. The medical record search mode can be text or exact. The latter requires the full structure of the electronic medical record.
Currently, the electronic medical record editors in China are developed based on text editors, which have major defects in processing data elements and structuring. The data structure of electronic medical records is not a simple matter of simply dividing the medical records into several pieces for writing. To a certain extent, the written content must be structured to distinguish the written content and guide the writing by the writer, it does not affect the description in natural language.
3. Security Management: according to the requirements of the Ministry of Health, the electronic medical record system must be able to implement electronic signatures, data traces, and other functions.
Iii. Technical Route
The commercialized electronic medical record editor in China provides the following technical directions:
1. Development Based on Delphi + richview
Richview is a Delphi-based text editing control. The development language is Pascal, and all source code is provided after the fee ($499.00. Companies that use this control to develop electronic medical records include medical records, yixun, and medical stars.
2. Development based on VC ++ abiword
Abiword is an open-source text editor control. The development language is C ++ and can be run across platforms, including all source code. The company that uses this control to develop electronic medical records is mainly emrpad. Many products of his manufacturers in China are OEM products or implemented based on their controls, such as Zoomlion.
3. Development Based on the control of the winning electronic medical records
The winning electronic medical record control is an office component that has been pushed to the electronic medical record market in recent years. In terms of text editing, its functions are the most complete and powerful, but the biggest defect of the company is that it is not a professional medical information manufacturer, but a provider of Text Editor controls, e-medical record manufacturers cannot obtain the source code, and there are many distribution and installation files, dozens of megabytes and hundreds of files.
4. other text editors
Some other vendors are using rich text editors such as Word, RichEdit, textcontrol, and scintilla, in general, there are problems such as difficulty in structured processing, implementation of continuous playing, and difficulty in writing control (such as control of copy and paste, and Expression Processing. There are also some manufacturers' products that cannot be seen due to limited personal capabilities, so it is difficult to assess.
The above points only represent personal opinions. If they are unfair, please correct me.
