The following exception was reported today when importing CSV to Oracle:

SQL*LOADER-522:LFIOPN failed for file

Later, from MSDN, the reason for folder permissions is insufficient .

Environment Description: My import file was placed on the WIN7 system desktop, which was copied from another successful import example on the desktop.

Processing scheme: copied to the other disk.
