ORA-15032 & ORA-15033 Solutions

Symptom description:
The following error message is returned when the ASM storage management cannot join the disk:
SQL> alter diskgroup disk add disk '/dev/rdsk/c2t500601603022E66Ad13s6 ';

ORA-15033: disk '/dev/rdsk/c2t500601603022E66Ad13s6' belongs to diskgroup "DISK"
 
Problem Analysis:
The raw device file header information is messy.
 
Solution:
Use dd to clear the header information of the bare device:
# Dd if =/dev/zero of =/dev/rdsk/c2t0d13s6 bs = 512 count = 562526688
Add the group after completion.
