If the version of Photoshop is cc, you can copy a layer of rounded rectangle 1 directly and then ctrl+t the size of the transform. The previous version of CC loses the fillet when it is distorted, so only shapes can be created.

Add the inner shadow and projection to the rounded rectangle 2.

Overlay a 10% black and white gradient for the "Wood copy" layer.

It is recommended that you ctrl+g this circle for the sound hole, because there will be many layers later.

However, if you look closely, you will find the wood grain of this icon still have problems, so finally to do a simple perspective.

The first is to choose the more bright wood grain, ctrl+t, right button Select "Perspective", adjust to this appearance.

Then use the same method to adjust the darker wood grain.
