Online summary three points cause this problem occurs: 1, the pin of the reset button does not correspond well, resulting in an error, 2, to the SDRAM CLK, and CPU CLK no lag-75 phase; 3, if the PLL output CLK, do not directly fly to the PLL input port, otherwise it will error. This communication error is basically due to a PIN definition error or a reset error. I summarize this mainly look at two aspects: Reset and CLK: (a) Reset pin binding is correct, and the chip model is correct. (b) CLK corresponds to the clock frequency is correct, compared to transplant is 20M, octave ration: phase after-75. The actual use is 50M. Online has said the chip revision version is the 4th version, the speed is 7.5ns (PC133 cl=3). At 50M, the PLL offset is-40.  Low-speed conditions do not offset can also. Second, Nios II has several files developed in win to set the compatibility mode, the unused pin to be set to three states is also a small reason that often causes errors.
