Analysis:
Error code 3221225477 to 16 is C0000005, which is a general protection error issued by the Windows core. So, as long as Apache itself or the use of the DLL (if you run PHP inline, but also including PHP and its use of the 3rd party DLL or so) crashes, will produce this win error.

Possible causes:
1, did not copy the LibmySQL.dll to the Windows/system32 directory. In fact, the answer is not so simple, it is likely that in the PATH environment variables corresponding to the search path already has a priority call and incompatible with the current Apache version of the LibmySQL.dll or other DLLs.
