Use the Type tool to enter the following text and get Layer 1.

Use the Rectangle tool to create a selection.

Double-click the text layer, open the Layer style panel, and set the color overlay option as follows.

Copy layer, get Layer 2, drag below layer 1, perform filter/blur/motion blur, parameter set as follows:

Press CTRL to click Layer 1, get the bit selection, and then subtract the following selection with the rectangular marquee tool.

Create new Layer 3, fill with white, and change opacity. Then enter the following small text.
